公費流感疫苗

◊本活動為免費 - 公費流感疫苗注射活動-疫苗以當日衛生所配發疫苗為主，無法指定廠牌

日期 : 2023/11/1 星期三

時間: 10:00 ~ 11 : 30 (依報名梯次報到→填疫苗同意書→醫師評估→安排疫苗注射)

地點: 大禮堂二樓-會議室(交大校區) 請由靠近郵局側樓梯上二樓

對象: 50歲以上 本校教職員工(參加者務必攜帶健保卡 )

報名網址: 10/23 中午12:30開放報名，請至陽明交通校內單一入口-陽明交通大學-學務處-健康自主管理系統-活動預約-活動報名(10/26中午12點或額滿截止)

★該活動以服務本校教職員工為主,恕不提供 -眷屬 /小兒 疫苗注射服務,敬請見諒

★主辦單位保有隨時修改及終止本活動之權利，若後續有任何變更內容或注意事項將公布於本組網頁

◊如需取消注射，請務必於活動前三天來電告知，避免疫苗浪費。
◊如有身體不適或COVID-19/流感相關症狀者，請在家休息，請勿參與活動。

◊ This event is a government funded influenza vaccine injection event.
◊ Vaccines are primarily distributed at the local health clinic on the same day and cannot be specified by brand.

Date: Wednesday, November 1, 2023

Time: 10:00 to 11:30 
(Participants should arrive according to their registration times→ Fill out the vaccine consent form → Medical evaluation by a physician → Arrange for vaccine injection)

Location: Second floor of the Grand Auditorium - Meeting Room ( Chiao Tung Campus). Please use the stairs located near the post office to access the second floor.

Eligibility: Faculty and staff  aged 50 and above at our school (Participants must bring  health insurance card)

Registration Website: Registration  opens at 12:30 PM on October 23. Please visit the NYCU Portal -> NYCU Campus -> Health Management Systems -> Event Reservation -> Event Application. (Registration will be closed when quota is full).
